Overview
You have diabetes or high blood pressure. When you apply for health insurance, you find out that most plans require you to wait 1 to 3 years before covering anything related to that condition. That is a long time to be left without any coverage.
This is exactly why insurers built waiting period reduction add-ons. They let you pay a bit more upfront, so chronic, common conditions get covered almost immediately, rather than years later.
In this article, we break down popular add-ons from HDFC ERGO, Care, Aditya Birla, Niva Bupa, ICICI Lombard, Star Health, and Tata AIG. We cover what each one reduces, what it costs you, and who genuinely needs it.
What Are Waiting Periods in Health Insurance?
A waiting period is the time you must wait after buying a policy before you can claim under certain conditions. Insurers apply this to prevent people from buying a policy only after they already need treatment. Here is the list of standard waiting periods.
Waiting period add-ons target this PED waiting period specifically, and sometimes only for a named list of lifestyle conditions. This can be reduced to 0 days, 30 days, or 1-2 years, depending on the case. Some plans, like ICICI Lombard Elevate, do offer the option to reduce the waiting period for specific illnesses, but the options are limited.
HDFC Optima Secure+ ABCD Chronic Care
The ABCD add-on in HDFC ERGO Optima Secure Plus reduces the PED waiting period for asthma, blood pressure, cholesterol, and type 2 diabetes from 3 years to just 30 days. Once this add-on is selected, there will be no loading charges for the selected conditions, and it is available to all members until age 65.
One Catch: Once you opt for ABCD, it cannot be removed for the life of the policy. It renews automatically every year with your base plan. This increases your overall cost over the years once the waiting period is completed.
Let’s have a look at the premiums, including the ABCD Chronic Care add-on, for some common profiles.
Note: A stands for adult and C stands for child. The premiums are calculated for a ₹15 lakh SI for individuals with Hypertension residing in Delhi (110010). Please note that these are indicative premiums, and issuance of the policy along with the rider will depend on medical underwriting.
Care Supreme Instant Cover and Instant Cover Plus
Care Supreme offers three separate add-ons that can help reduce waiting periods. The benefit of these add-ons is that they can be removed after 3 years, so that you don’t have to bear the extra cost. You can opt for either one of the add-ons, not all of them together.
- Instant Cover: This paid add-on reduces the waiting period for hypertension, diabetes, hyperlipidemia, and asthma from 3 years to 30 days.
- Instant Cover Plus: This add-on reduces the waiting period for 7 listed conditions from 3 years to 30 days. The listed conditions include Instant Cover’s 4 conditions plus Chronic Obstructive Pulmonary Disease (COPD), Obesity, and coronary artery disease with Percutaneous Transluminal Coronary Angioplasty (PTCA) done prior to 1 year.
- Reduction in PED: The above add-ons reduce waiting times for the listed conditions. For any diseases apart from the list, this add-on reduces the waiting period from 3 years to either 1 / 2 years.

Aditya Birla Activ One MAX Chronic Care
This add-on waives the PED waiting period entirely, bringing it down from 3 years to 0 days, for 7 chronic conditions, including diabetes, asthma, hypertension, cholesterol, COPD, obesity, and coronary artery disease (with prior PTCA).
Activ One MAX also offers a separate reduction in PED add-on to cover any other pre-existing disease, reducing the standard 3-year wait to 2 or 1 year. Both riders can be taken together based on each individual's medical profile.
Please Note: Once taken, add-ons cannot be removed for the entire policy term.

Niva Bupa ReAssure 2.0 Disease Management
The Niva Bupa ReAssure 2.0 Disease Management rider offers day 1 coverage, meaning zero waiting period, but only for 2 conditions, including diabetes and Hypertension. It is narrower than most plans on this list.
For any other PEDs, the standard 3-year waiting period is applicable. If you’re looking to cover parents or senior citizens with these two conditions, this add-on makes sense. If not, you can consider waiting and using your corporate cover for the first 3 years.
ICICI Lombard Elevate Jump Start
Jump Start reduces the PED wait to 30 days for 6 conditions: asthma, diabetes, hypertension, hyperlipidemia, obesity, and coronary artery disease (PTCA done prior to 1 year), provided you declare them at purchase. It must be chosen for 3 consecutive policy years.
ICICI Lombard Elevate also lets you shorten the maternity waiting period from 2 years to 1 year through its dedicated Reduction of Maternity Waiting Period rider.
Did You Know?
Star Super Star Quick Shield
Quick Shield waives the PED wait for hypertension, diabetes, asthma, hyperlipidemia, and coronary artery disease, covering them from day 31. It is only available to buyers up to age 65.
Star Super Star also offers a separate Reduction in PED Waiting Period add-on, which brings the standard 36-month wait down to 12 or 24 months for any condition and reduces the Specified Disease Waiting Period from 2 years to 1 year.
Tata AIG Medicare Select Advanced Cover Rider
The Advanced Cover rider reduces the waiting period for type 2 diabetes, hypertension, hyperlipidemia, and asthma to just 30 days, down from 3 years.
Tata AIG Medicare Select also offers a separate Reduction of Maternity Waiting Period rider, which reduces the maternity waiting period from 2 years to 1 year.

Who Should Buy a Waiting Period Add-On?
Anyone Managing Chronic Conditions
Someone Relying Mainly on Corporate Policy
Whose Condition Fits the Named List
You Plan to Stay With the Insurer Long-Term
Bottom Line: These add-ons work best for genuine chronic PEDs, where the wait drops from 3 years to 30 days or less. If a condition also falls under the specific illness list, such as cataracts or a hernia, the add-on doesn't provide much value, since the wait is already capped at 2 years. The gap to close is too small to justify the extra premium.

Conclusion
Waiting period add-ons solve one specific problem. They get a named, common condition covered sooner, anywhere from day one to 1 or 2 years earlier, instead of the standard 3-year PED wait.
That's the entire decision to make. Match your actual diagnosis to the conditions listed under each add-on, not the insurer's brand name.
Before you add one to your policy, do this:
- Check the specific illness list first. Pull out your existing or proposed policy wording. A 2-year cap there might already cover what you need.
- Compare the cost against the risk. Weigh the add-on premium against 3 years of out-of-pocket risk for that one condition.
- Confirm the lock-in. Check whether the add-on remains for the policy's lifetime, as that affects your long-term cost.
Get this right once, at the time of purchase, and you won't need to revisit it at every renewal.
