Restoration, Bonuses, and Add-Ons
Restoration
HDFC ERGO Optima Secure restores 100% of your base sum insured after it gets used up in a claim, even if it is partially exhausted. This restored amount can be used for both related and unrelated illnesses, but unlimited restoration is available only with the add-on. It usually costs very little, so it is worth considering if you want stronger support for restoration.
Niva Bupa Aspire works differently. Its ReAssure Forever benefit gives you unlimited restoration after your very first claim is paid, with no add-on needed. It applies to both related and unrelated illnesses, and even in cases of partial exhaustion.
Bonus
HDFC ERGO Optima Secure gives a 50% to 100% bonus on the SI. This is added to coverage irrespective of claims.
Niva Bupa Aspire offers the Booster+ benefit. Any portion of your base SI that remains unused gets carried forward and added to next year's cover at renewal. Unlike a typical no-claim bonus, Booster+ keeps building even in years you've claimed, up to a specified limit. How much it grows depends on the age of the eldest member at the time you bought the policy.
Add-Ons
Apart from Unlimited Restoration, HDFC ERGO Optima Secure offers the following add-ons:

Optima Wellbeing
Adds Outpatient Department (OPD) coverage with unlimited doctor visits, both in-clinic and online. You also get free home sample collection, wellness sessions, and up to 50% off on tests, check-ups, and 20% off on medicines. This works on a cashless basis through partner facilities only in limited cities.
Limitless
Removes the upper limit on your medical expenses, within set conditions. If your base sum insured is between ₹10 lakh and less than ₹50 lakh, you get one infinite-value claim over the policy's lifetime. For ₹50 lakh and above, you get two such claims.
ABCD Chronic Care
Covers hospitalization for asthma, high blood pressure, cholesterol, and type 2 diabetes after just a 30-day wait, down from the usual 3-year Pre-Existing Disease (PED) waiting period.
Parenthood
Covers maternity expenses after a 2-year wait, including up to 2 deliveries or terminations, pre- and post-natal care within 180 days of childbirth, embryo freezing, and up to 2 In-Vitro Fertilization (IVF) cycles. Newborn cover is not included.
Please Note: It is always advisable to check the availability of add-ons before purchasing any plan.
Niva Bupa Aspire takes a more flexible, lifestyle-driven approach to add-ons:
- Safeguard+: Covers non-payable items, lowering your out-of-pocket costs. Booster+ stays intact if claims in a policy year stay under ₹1 lakh. It also bumps up your base SI every year to keep pace with inflation.
- Fast Forward: Lets you buy a 2-year or 3-year policy and pool your entire sum insured, including base and M-iracle, into one cover. You can use this total cover at any time during the policy term.
- Future Ready: Guarantees your future spouse gets added to the policy, carrying forward any waiting periods you've already served. You just need to add them within 90 days of marriage.
- Disease Management Rider: This add-on helps reduce the pre-existing disease waiting period for diabetes and high blood pressure from 3 years to 0 days.
- WellConsult (OPD): Covers unlimited teleconsultations, up to ₹10,000 in physical consultations, ₹10,000 in diagnostics, and ₹2,500 in pharmacy costs each year, at a 20% co-pay if opted for reimbursement. It also includes emotional wellness sessions, fitness coaching, and limited gym access.
Please Note: This is not an exhaustive list of add-ons. Please check the insurer's brochure and policy wording for complete details.
HDFC ERGO vs Niva Bupa: Performance Metrics
Note:
- The above-mentioned information is sourced from the IRDAI annual reports, Ditto Data Lab, HDFC ERGO, and Niva Bupa public disclosures.
- The ICR and industry average have been considered for FY 2023-25, while other metrics have been updated for FY 2024-26.
Key Insights:
- Claim Settlement Strength: HDFC ERGO demonstrates stronger claim-settlement consistency, comfortably ahead of the industry average, indicating faster, smoother settlements. Niva Bupa also performs above average, though it trails HDFC ERGO by a noticeable margin.
- Customer Service Experience: HDFC ERGO reports a fraction of the complaints seen industry-wide, indicating smoother service and fewer customer grievances. Niva Bupa's complaint levels run well above the industry average, suggesting more service-related friction.
- Financial Health: HDFC ERGO's ICR falls within the healthy range (55%-85%), suggesting that payouts are broadly in line with premiums collected. Niva Bupa's ICR is lower than average but still within the healthy range, indicating a financially stable insurer that pays out conservatively.
- Market Size and Network Access: Both insurers have built sizable health insurance businesses with premium collections well above the industry average. HDFC ERGO edges ahead on hospital network size, giving it wider cashless access across the country.
Which One Should You Choose?
At Ditto, HDFC ERGO Optima Secure is recommended out of the two. It works well for buyers who prioritize claim reliability above all else. On the other hand, Niva Bupa Aspire is a good pick for younger families planning pregnancy within the next year, owing to its shorter maternity waiting period.
Please Note: If maternity is the sole reason you're considering health insurance, pause for a moment first. At Ditto, we generally don't recommend buying health insurance only for maternity coverage. Even with shorter waiting periods, the coverage limits often fall short of real delivery costs, and the premium rarely pays for itself on that benefit alone. A corporate health policy or a dedicated savings buffer usually works out better here.

Conclusion
Both HDFC ERGO Optima Secure and Niva Bupa Aspire are strong plans, but they serve different needs.
Optima Secure wins on the fundamentals with a 97.61% CSR, day-one cover doubling, just 8.87 complaints per 10,000 claims, and a hospital network of 16,000+. If claim reliability and predictable cover growth matter most to you, this is the cleaner choice.
Aspire makes sense if you're planning a family soon. Its Titanium+ variant covers maternity, IVF, and newborns after just a 9-month wait, something Optima Secure does not offer in the base plan. Maternity depends on the availability and terms of the Parenthood add-on. It also comes at a lower premium for senior citizens and gives you more room to customize through add-ons.
For most buyers focused on claim confidence and simplicity, HDFC ERGO Optima Secure is the stronger pick. For young families prioritizing maternity cover, Niva Bupa Aspire is hard to beat. If you're still unsure, refer to our comprehensive guide to the best health insurance plans in India.
