Care Ultimate vs. HDFC ERGO Optima Secure Plus

Overview

Care Ultimate offers wider flexibility for pre-existing conditions and several loyalty-linked benefits, while HDFC ERGO Optima Secure Plus focuses on stronger built-in cover growth and insurer-level performance.

Core Coverage & Differences

  • Sum Insured Growth: Optima Secure Plus starts at 2X cover and adds 100% of the base cover every year without a cap. Care Ultimate starts at 1X, with uncapped growth available through the paid Infinity Bonus add-on.
  • Restoration: Both plans offer unlimited restoration by default.
  • Pre-Existing Diseases: Care can reduce waiting periods to 30 days for seven listed conditions, versus four under HDFC ERGO’s ABCD Chronic Care add-on.
  • Extra Benefits: HDFC includes consumables cover and offers maternity as an add-on, while Care offers Premium Payback and other loyalty-linked benefits.

Insurer Performance & Reliability

  • Claims & Complaints: HDFC ERGO has a higher claim settlement ratio, fewer complaints, and a wider hospital network than Care Health.

Imagine you're 28, buying your first health insurance, and you land on two plans that both promise your sum insured will keep multiplying over time. Optima Secure Plus says your cover doubles from day one and never stops growing. Care Ultimate, meanwhile, gives you multiple add-on options to increase your cover as much as you want.

Both sound great, but the real difference between Care Ultimate vs. HDFC ERGO Optima Secure Plus shows up only once you dig into the fine print.

In this comparison, we'll discuss the key features, including how bonus and restoration work, how much each plan costs, and how the two insurers, Care Health and HDFC ERGO, perform on claims and complaints.

How the Two Plans Grow Your Cover: Infinite Benefit Versus Infinity Bonus

HDFC ERGO Optima Secure Plus

Background Image

01

1X Base Cover

You choose your sum insured, say ₹10 lakh, and premiums are calculated on this amount alone.

02

2X Secure Benefit

The moment your policy starts, your cover doubles to ₹20 lakh, with no waiting period and no extra premium.

03

Infinite Benefit

At every renewal, 100% of your base sum insured is added to your total cover, with no upper limit, whether or not you made a claim the previous year.

So a ₹10 lakh base plan gives you ₹20 lakh of cover from day one. After 10 years of continuous renewal, that same plan can offer roughly ₹1.2 crore in total cover (₹10 lakh base, plus the ₹10 lakh Secure Benefit, plus ₹1 crore built up through the Infinite Benefit).

Key Insight: This is a real structural upgrade over the plain HDFC ERGO Optima Secure plan, whose equivalent Plus Benefit is capped at 100% of the base sum insured. A ₹10 lakh Optima Secure policy maxes out closer to ₹30 lakh over the same 10-year stretch (base, Secure Benefit, and Plus Benefit combined), while Optima Secure Plus keeps compounding well past that.

Care Ultimate

The Care Ultimate plan takes a different route. There's no day-one multiplier, so you start at 1X of your chosen sum insured, and growth happens through a mix of built-in and paid features:

Background Image

01

Cumulative Bonus (Built-In)

Your sum insured grows by 50% every renewal year, capped at 100% of the base. So a ₹10 lakh plan reaches ₹15 lakh in year 2 and ₹20 lakh in year 3, and then this benefit stops increasing.

02

Loyalty Boost (Built-In)

After 7 consecutive claim-free years, Care adds a one-time bonus equal to 100% of your first year's base sum insured, on top of whatever you've already built up. This applies once in the policy's lifetime.

03

Infinity Bonus (Add-On)

This removes the 100% cap on the Cumulative Bonus entirely. Your sum insured grows by 100% every year you renew, with no upper limit, as long as you continue paying for the add-on.

04

Tenure Multiplier (Add-On)

If you pay for a multi-year policy upfront, this combines your sum insured across the remaining policy years into one large claim amount, useful if you're ever facing a single very high-cost treatment.

Key Insight: Optima Secure Plus's growth is guaranteed and built into the base premium from day one. Care Ultimate's fastest growth path, the Infinity Bonus, is an add-on you pay for every year, so weigh the long-term add-on cost against the guaranteed, no-extra-cost growth that Optima Secure Plus already offers.

Pre-Existing Disease Waiting Periods Compared

Both plans impose the standard 3-year Pre-Existing Disease (PED) waiting period by default, and both let you shorten it through add-ons, but the details differ.

Care Ultimate: Instant Cover

    • The Instant Cover add-on brings the waiting period down to just 30 days for seven listed conditions. These include diabetes, hypertension, hyperlipidemia (high cholesterol), asthma, hypothyroidism, obesity, and coronary artery disease (if angioplasty was done more than a year ago).
    • A separate Reduction in PED add-on can bring the wait down to 1 or 2 years for any other pre-existing condition. If the condition is on the specific illness list, the waiting period can be reduced to 2 years, not 1 year.
    • You can select one of the two add-ons above. You need to pay the extra premium for the add-on only for the first 3 years.

Optima Secure Plus: ABCD Chronic Care

    • The ABCD Chronic Care add-on reduces the waiting period to 30 days, but only for four conditions: asthma, blood pressure (hypertension), cholesterol, and type 2 diabetes on oral medication.
    • You must pay the extra premium for the add-on for the entire policy lifetime.

Key Insight

If you or a family member already has a pre-existing condition, Care Ultimate health insurance offers more flexibility. Its Instant Cover applies to seven listed conditions, compared with four under Optima Secure Plus’s ABCD Chronic Care, and Care also offers a separate PED reduction add-on for other illnesses.

Another important difference is underwriting. Care Ultimate does not levy a premium loading specifically because of a PED, while HDFC ERGO can apply an underwriting loading depending on your medical history. If it imposes such a loading, you continue paying it for as long as the policy remains in force.

So, for someone entering the policy with an existing medical condition, Care Ultimate can be more flexible both in reducing waiting periods and in keeping the long-term premium impact lower.

Ditto's Observation on Premiums

Care Ultimate isn’t consistently cheaper even with its first-year Welcome Discount. In these samples, it costs less only for the ₹15 lakh cover at ages 25 and 32/31, while Optima Secure Plus is cheaper in the other profiles, especially at ₹25 lakh. Care’s 30% Welcome Discount drops to 20% in year two, 10% in year three, and disappears in year four, so its long-term premium could be meaningfully higher than the first-year quote suggests.

Incurred Claims Ratio (ICR) (Average FY 2023-25)58.68%81.62%HDFC ERGO's ICR sits closer to the upper half of Ditto's ideal 55% to 85% band, suggesting a more generous payout ratio against premiums collected.
Volume of Complaints per 10,000 Claims42.678.87HDFC ERGO records under a quarter of Care's complaint volume, a meaningful gap.
Annual Business Volume / GWP (Total Health)₹8,405 crore₹6,597 croreCare Health has a larger health insurance premium base, reflecting a bigger health portfolio.
Network Hospitals11,400+16,000+Both comfortably clear Ditto's 10,000+ recommended benchmark, but HDFC ERGO's network is meaningfully wider.

Care Health is a standalone health insurer, so all its premium and complaint volume comes purely from health policies. HDFC ERGO is a general insurer, so its complaint figures span motor, fire, and other lines too, which matters when comparing scale.

On insurer fundamentals alone, HDFC ERGO is the stronger pick. For more, see our detailed Care Health Insurance review and HDFC ERGO Health Insurance review.

Pros and Limitations of Care Ultimate

Pros

    • Unlimited Automatic Recharge is built into the base plan, covering multiple hospitalizations in the same policy year.
    • Instant Cover offers a 30-day waiting period for seven listed pre-existing conditions, and a working "any illness" PED reduction add-on.
    • The base plan has no room rent cap and no mandatory co-payment.
    • Premium Payback refunds your first year's base premium after every block of five consecutive claim-free years, a genuine reward for staying healthy.

Limitations

    • There's no day-one cover multiplier. You start with just the base sum insured you chose.
    • Consumables need the paid Claim Shield add-on.
    • Most of the plan's headline features, including Infinity Bonus, Premium Payback, and Tenure Multiplier, cost extra.
    • The Welcome Discount steps down from 30% in year one to zero by year four, so the first-year premium understates the plan's real long-term cost.
    • Maternity isn't offered, not even as a paid add-on.
    • Care's complaint rate is materially higher than HDFC ERGO's.

Pros and Limitations of HDFC ERGO Optima Secure Plus

Pros

    • The Secure Benefit doubles your base cover from day one, at no extra premium.
    • The Infinite Benefit grows your cover by 100% every year, with no upper limit, even after a claim.
    • Unlimited restoration is built in, not sold as an add-on.
    • The Protect Benefit covers listed consumables without needing a separate rider.
    • HDFC ERGO has a higher claim settlement ratio, a lower complaint volume, and a larger cashless hospital network.
    • Maternity is available through the Parenthood add-on.

Limitations

    • Entry age is capped at 60 for new buyers, so it isn't an option for older parents.
    • There's no Premium Payback or wellness-linked discount, something a few competitors already offer.

Which Plan Should You Choose?

Choose Care Ultimate if

    • You or a family member manages one of the seven listed pre-existing conditions and want the widest Instant Cover list.
    • You want a plan that rewards a long claim-free streak with an actual premium refund.
    • You're above 60 (since Optima Secure Plus isn't available past that age).
    • You're comfortable paying for add-ons in exchange for uncapped bonus growth and multi-year flexibility.

Choose HDFC ERGO Optima Secure Plus if

    • You want high, guaranteed cover from day one without waiting for bonuses to build up.
    • You want consumables covered without paying extra.
    • You're under 60 and planning to hold the policy long term.
    • You'd rather choose the insurer with the stronger claims and complaints track record.

Ditto's Take

Both plans are genuinely competitive, but they reward different things.

HDFC ERGO Optima Secure Plus hands you a large, guaranteed cover on day one, backs it with unlimited restoration built in rather than sold as an add-on, and is supported by an insurer with better claims and complaints metrics.

Care Ultimate asks you to stack bonuses and pay for some of its best features separately, but it gives you a wider net for pre-existing conditions, a genuine reward for staying claim-free through Premium Payback, and no upper age limit on entry.

Before you buy either, check what's actually guaranteed versus what depends on a claim, an add-on, or a discount that will eventually run out. You can also explore our best health insurance plans in India for more options.

Frequently Asked Questions

Which is better overall, Care Ultimate or HDFC ERGO Optima Secure Plus?

It depends on what you value most. On product features, Care Ultimate edges ahead slightly, thanks to its wider pre-existing disease coverage, Premium Payback, BeFit benefit, and Tenure Multiplier. On insurer track record, HDFC ERGO is the clearer winner, with a higher claim settlement ratio, far fewer complaints, and a larger hospital network. If you're comparing purely as an insurer, HDFC ERGO comes out ahead, but if you're comparing plan-level perks, Care Ultimate has a slight edge.

Does HDFC ERGO Optima Secure Plus really grow cover with no upper limit?

Yes. The Infinite Benefit adds 100% of your base sum insured to your total cover every year at renewal, with no cap, whether or not you made a claim the previous year. Combined with the 2X Secure Benefit on day one, a ₹10 lakh base plan can realistically grow to over ₹1 crore in total cover after about 10 years of continuous renewal.

Which plan covers diabetes sooner, Care Ultimate or Optima Secure Plus?

Both can bring the waiting period for diabetes down to 30 days through an add-on. Care Ultimate's Instant Cover add-on covers diabetes along with six other conditions, including hypertension and asthma. Optima Secure Plus's ABCD Chronic Care add-on also covers diabetes, but only alongside three other conditions: asthma, blood pressure, and cholesterol.

Is Optima Secure Plus worth the extra premium over plain Optima Secure?

Usually, yes. The premium difference is small, around ₹13,459 versus ₹13,426 annually for a 25-year-old at ₹15 lakh cover, but the difference in long-term coverage is significant. Plain Optima Secure's bonus caps at 100% of the base sum insured, while Optima Secure Plus's Infinite Benefit has no ceiling. For buyers planning to hold the policy for a decade or more, that gap is usually worth the marginal extra cost.

Can I port from Care Ultimate to HDFC ERGO Optima Secure Plus without losing waiting period credit?

Waiting periods already served under a previous policy are generally carried forward during portability, as per IRDAI's portability regulations, up to your existing sum insured and any bonus earned. That said, HDFC ERGO currently places some restrictions on porting into Optima Secure Plus specifically, and any increase in sum insured or new members added will attract fresh waiting periods. It's worth confirming the exact terms with the insurer or a Ditto advisor before you apply.

Which of the two plans leaves fewer non-payable deductions at claim time?

Optima Secure Plus, since its Protect Benefit covers 68 listed consumables (like gloves, syringes, and PPE kits) as a built-in feature. Care Ultimate can offer the same protection, but only if you've opted for the paid Claim Shield add-on at the time of purchase. If you forget to add it, you may have to pay for these items out of pocket.

Does Care Ultimate or Optima Secure Plus cover maternity?

Care Ultimate plan doesn't offer maternity cover, not even as an add-on. Optima Secure Plus offers it through the Parenthood add-on, which covers ₹50,000 to ₹2 lakh in maternity expenses after a waiting period, along with pre- and post-natal care. If maternity cover is a priority, Optima Secure Plus is the only one of the two that offers a path to it.
