Cashless vs. Reimbursement Claims: What to Track and When
For Cashless Claims
Under cashless treatment, Care Health Insurance pays eligible medical expenses directly to the hospital after authorization. The hospital coordinates with the insurer and submits the required medical documents.
As per IRDAI guidelines, insurers must decide on initial cashless pre-authorization requests within 1 hour and provide final discharge authorization within 3 hours of receiving the hospital's discharge authorization request.
If final discharge authorization takes more than three hours, the insurer must bear applicable additional hospital charges caused by the delay. Keep a record of when the hospital submitted its discharge authorization request and when you received approval.
Note: These deadlines apply to approving the request, not to transferring money to the hospital.
What to Track: Check whether pre-authorization has been approved, whether Care has requested additional medical information, and whether the hospital has issued final discharge authorization. Also confirm any deductions and expenses you must pay.
If approval is delayed, contact the hospital's insurance desk and Care's customer support team.
For Reimbursement Claims
To check your Care Health Insurance reimbursement claim status, track your claim after you pay the hospital bills and submit the required documents to Care Health Insurance for assessment and payment.
Care's published service information states that reimbursement claimants should receive a status update within 7 days of claim registration, through SMS, WhatsApp, or email. Care states that reimbursement claims should generally be settled within 15 days of submission. This is separate from the seven-day status update. Incomplete documents or additional verification requirements may affect processing.
What to Track: Check whether your claim has been registered, whether any documents or clarifications are pending, and whether Care has communicated its decision. Once approved, check your bank account for the payment.
Documents Required for Care Health Insurance Claims
The following lists cover commonly required documents and are indicative, not exhaustive. Requirements may vary by claim type and policy. Care supports uploading scanned documents through its official portal or app. Keep the original documents available in case the insurer requests them for verification.
Documents Required for Cashless Claims
- Care Health Insurance policy document, health card, or e-health card.
- A valid government-issued photo ID for the patient.
- Completed and signed pre-authorization form.
- Doctor's prescription or consultation papers, admission advice, and relevant diagnostic reports.
Documents Required for Reimbursement Claims
- A completed and signed reimbursement claim form.
- Original hospital discharge summary.
- The original final hospital bill showing individual charges, along with payment receipts.
- Doctor's prescriptions, diagnostic test reports, and pharmacy bills related to your treatment.
- Certified copies of your hospital treatment records, including notes recorded during your stay.
- Implant invoice and sticker, if applicable.
- Canceled cheque with the account holder's printed name. For individual or family floater policies, the cheque should be in the proposer's name. For corporate policies, it should be in the employee's name.
- KYC documents showing identity and address for claims of ₹1 lakh or more. The address should match the one on the policy.
- For accidents, a Medical Legal Case (MLC) report or First Information Report (FIR), where applicable.
What to Do If Care Health Insurance Rejects Your Claim?
- Understand the Reason: Read the rejection letter to understand why the claim was denied. Check the cited policy clauses, such as waiting periods, exclusions, or missing documents.
- Gather Supporting Documents: Collect medical reports, prescriptions, hospital bills, and any clarification from your treating doctor that may help address Care's concerns.
- Request a Review: Contact Care customer support at 1800-102-4488 or customerfirst@careinsurance.com and request a written explanation. If the issue remains unresolved, escalate it to Care's Grievance Redressal Officer through the contact details published on Care's official website.
- Escalate the Matter: If Care does not resolve your complaint within the required time, or you're unhappy with its response, register a complaint through the Bima Bharosa portal.
- Approach the Insurance Ombudsman: If your grievance remains unresolved after 30 days or you are dissatisfied with Care's response, you may approach the independent Insurance Ombudsman, subject to eligibility requirements. The claim amount, including expenses, must not exceed ₹50 lakh, and you must file the complaint within the prescribed period.
