Overview

The Rashtriya Swasthya Bima Yojana (RSBY) was a government-funded, cashless health insurance scheme launched for Below-Poverty-Line (BPL) families in the unorganized sector. It has since been merged into Ayushman Bharat Pradhan Mantri Jan Arogya Yojana (PM-JAY).

The scheme offered:

  • Family Floater Cover: Health insurance of up to ₹30,000 per family per year.
  • Eligible Members: Up to 5 family members, including the head of household, spouse, and 3 dependents.
  • Cashless Benefits: Covered hospitalization expenses, including medicines, along with a transport allowance of ₹100 per hospitalization.
  • Affordable Enrollment: Beneficiaries paid a ₹30 annual registration fee, while the central and state governments shared the premium cost.

Although RSBY is no longer active, eligible families now receive higher benefits under Ayushman Bharat PM-JAY through a wider network of hospitals. Eligibility is based on SECC 2011 data, but also extends to enrolled RSBY families not listed in SECC, and to all citizens aged 70 and above, irrespective of income.

India's push toward universal, cashless health coverage didn't start with Ayushman Bharat. It began over a decade earlier, in 2007, when the government launched the Rashtriya Swasthya Bima Yojana (RSBY) to provide below-poverty-line households with access to hospitalization without having to pay upfront. The scheme's cashless, portable, paperless design became the template on which PM-JAY was later built, before RSBY itself was subsumed into that larger scheme in 2018.

This guide walks you through the Rashtriya Swasthya Bima Yojana card benefits, how its eligibility worked, how it evolved into PM-JAY, and exactly where to check your status now

What Is Rashtriya Swasthya Bima Yojana?

Rashtriya Swasthya Bima Yojana (RSBY) was a centrally sponsored health insurance scheme launched by the Ministry of Labour and Employment, Government of India. It provided cashless health insurance to identified beneficiaries in the unorganized sector, mainly households below the poverty line.

To enroll, beneficiaries paid a nominal ₹30 registration fee for a biometric-enabled smart card carrying their fingerprints and photographs. This card was used to access up to ₹30,000 in inpatient medical care per family per year at any empaneled hospital, with an additional provision for transport expenses.

The premium itself was funded jointly by the Center and the states, with the Center contributing 75% of the estimated annual premium and the state government covering the remaining 25%, a split that shifted to 90% Center and 10% state for the northeastern states and Jammu and Kashmir.

How Did RSBY Evolve Into PM-JAY?

  • 2007–08: RSBY was launched under the Ministry of Labour and Employment, covering BPL families in the unorganized sector.
  • 2011–2014: Eligibility expanded to 11 additional occupational categories.
  • April 1, 2015: The scheme moved from the Ministry of Labour and Employment to the Ministry of Health and Family Welfare.
  • April 1, 2016: The Senior Citizen Health Insurance Scheme (SCHIS) added a ₹30,000 annual top-up for citizens aged 60 and above.
  • 2018: RSBY and SCHIS were subsumed into Ayushman Bharat, which launched as PM-JAY on September 23, 2018. Cover increased from ₹30,000 to ₹5 lakh, and eligibility shifted from BPL lists to SECC 2011 criteria.

As of early 2026, RSBY no longer functions as a standalone program. Most states migrated existing beneficiary data into the PM-JAY database, and all new enrollments now run entirely through PM-JAY.

Rashtriya Swasthya Bima Yojana Card: Benefits and Coverage

Here's what the card actually offered while the scheme was active:

FeatureDetails
Annual Cover₹30,000 per family, on a floater basis
Family UnitHead of the household, spouse, and up to 3 dependents
Registration Fee₹30 for the biometric smart card
Pre-Existing DiseasesCovered from day one without any waiting period
Age LimitNone
PortabilityUsable at any empaneled hospital across India
Claims ProcessCashless and paperless at network hospitals
Additional BenefitsTransport expense provision included (₹100 per hospitalization, capped at ₹1,000 per year)
Top-Up for Senior Citizens₹30,000 extra per year under SCHIS (from 2016)

These Rashtriya Swasthya Bima Yojana benefits were considered generous for their time, particularly the absence of an age limit and day-one coverage for pre-existing conditions. Many private health insurance policies still don't offer these features. That said, the ₹30,000 family floater cover was minimal, especially considering hospitalization costs. This is one of the reasons the scheme was eventually replaced by PM-JAY, which offers a much higher ₹5 lakh cover.

Eligibility Criteria for Rashtriya Swasthya Bima Yojana

RSBY's eligibility rules evolved over its lifetime, expanding well beyond its original BPL-only mandate:

    • Original Eligibility (2008): Below-poverty-line (BPL) households, identified through state-level BPL lists, primarily in the unorganized sector.
    • Expanded Eligibility (2011–2014): 11 additional worker categories were added, including building and construction workers, street vendors, and domestic workers.
    • Senior Citizens (2016 Onward): Individuals aged 60 and above from BPL households or the 11 additional categories became eligible for the SCHIS top-up, in addition to their existing RSBY entitlement.

Since RSBY has been merged into PM-JAY, none of these eligibility criteria apply to new applicants anymore. If you're checking whether you qualify for government health cover today, eligibility is now determined by criteria based on the Socio-Economic and Caste Census 2011 under PM-JAY, rather than the old RSBY BPL lists.

How to Apply for the Rashtriya Swasthya Bima Yojana Card?

There is no active application process for a Rashtriya Swasthya Bima Yojana card. RSBY stopped accepting new enrollments once it was absorbed into PM-JAY in 2018. If you're eligible for government-funded health cover today, you'll need to apply through PM-JAY instead, which follows a similar but updated process:

  1. Through the PM-JAY Website: Visit the official PM-JAY portal, select "Am I Eligible," log in with your registered mobile number and OTP, and search for your beneficiary record using your state, district, Aadhaar number, family ID, or PM-JAY number. Complete Aadhaar-based e-KYC, and once approved, download your card directly from the same page.
  2. Through the Ayushman App: Install the official Ayushman app on Android or iOS, log in with your mobile number and OTP, and follow the same verification steps as on the website.
  3. In Person at a Common Service Centre (CSC): Visit your nearest Atal Seva Kendra or CSC, request card generation, verify your mobile number and OTP, and submit the required documents. Staff will process the request, and the card is typically ready within a few days.

If you were previously an RSBY beneficiary, it's worth checking with your state health department or a CSC to confirm whether your family's records have been migrated to the PM-JAY database, as this can sometimes speed up verification.

How to Check Rashtriya Swasthya Bima Yojana Status?

A Rashtriya Swasthya Bima Yojana status check today won't return an active enrollment confirmation because the scheme no longer exists as a standalone program. If your family was enrolled in RSBY and you're unable to find a matching record in PM-JAY, it's worth visiting your nearest CSC or Ayushman Arogya Mandir with your old RSBY card and identity documents, as manual verification is sometimes required for older records.

In contrast, other senior-only families receive ₹5 lakh per family through the Ayushman Vay Vandana Card.

The comparison makes it clear that PM-JAY isn't a wholly separate concept from RSBY. A parliamentary panel has actually described Ayushman Bharat as essentially a modification of RSBY. PM-JAY retained the cashless, portable, public-private empanelment approach that made RSBY workable, but scaled up coverage and modernized eligibility criteria to reach a much larger population.

Conclusion

Rashtriya Swasthya Bima Yojana played a genuine role in India's health insurance history. It was the first scheme to prove that a cashless, portable, paperless cover could work at scale for low-income households.

But it's been discontinued since 2018, with Ayushman Bharat PM-JAY taking over its role with much broader coverage and eligibility. If you're checking eligibility or status today, PM-JAY is where to start your search, not RSBY.

While government-funded, social-security-based health plans are excellent safety nets for the lower and middle classes, they often face tight budgets, policy changes, and operational delays. Therefore, individuals who prefer high-quality care, such as specific doctor choices, private rooms, and personalized coverage, should always invest in a comprehensive personal health insurance policy rather than relying solely on government schemes.

Frequently Asked Questions

Is Rashtriya Swasthya Bima Yojana still active for new enrollments in 2026?

No. RSBY stopped accepting new enrollments once it was subsumed into PM-JAY in 2018, and there is no active RSBY enrollment process in 2026.

What happens to an existing Rashtriya Swasthya Bima Yojana smart card holder today?

In most states, RSBY beneficiary records were migrated into the PM-JAY database. The old smart card itself is no longer valid for claims, so the holder needs to check eligibility and download a PM-JAY card through the current portal instead.

Can I check my old Rashtriya Swasthya Bima Yojana status on the official RSBY website?

No. The official RSBY website is no longer accessible, so there's no direct channel to check RSBY status. Eligibility and status should be checked through the PM-JAY portal instead.

Do I need to re-register for PM-JAY if I was previously enrolled in Rashtriya Swasthya Bima Yojana?

Not necessarily. Many RSBY records have already been migrated to the PM-JAY database, so it is worth checking eligibility on the PM-JAY portal first. If no matching record turns up, a visit to a nearby CSC or Ayushman Arogya Mandir with the old RSBY card and identity documents can help complete manual verification.

Should I rely only on RSBY successor schemes like PM-JAY, or also buy private health insurance?

Government schemes like PM-JAY are useful safety nets, but they often come with tighter budgets and a limited choice of hospitals or doctors. Anyone who wants more flexibility, such as private rooms or preferred specialists, should consider a personal health insurance policy alongside any government cover.

What was the maximum coverage amount provided under Rashtriya Swasthya Bima Yojana before it ended?

RSBY provided up to ₹30,000 per year in hospitalization cover for a family unit of five, on a floater basis.

Were pre-existing diseases covered from day one under Rashtriya Swasthya Bima Yojana?

Yes. RSBY covered pre-existing diseases from the very first day of enrollment, with no waiting period.

Why was RSBY replaced by Ayushman Bharat PM-JAY?

RSBY offered cashless hospitalization cover, but its ₹30,000 limit soon fell behind rising treatment costs, only a fraction of intended BPL households ended up enrolling, and outpatient expenses remained uncovered throughout. Instead of patching these gaps, the government built PM-JAY on RSBY's foundation, keeping its cashless, portable, public-private hospital model while significantly raising cover to ₹5 lakh and broadening eligibility.

What did the Rashtriya Swasthya Bima Yojana card actually cover medically?

RSBY covered hospitalization for illness or accidents at empaneled hospitals via pre-fixed package rates, including diagnostics and medicines one day before admission and five days after discharge. It also covered day care procedures, maternity care, newborn care, and accident-related dental treatment, all within the ₹30,000 annual family limit.
